VNUS Medical Technologies ("VNUS") makes the Physician Locator Service ("Service") available for your use on this Website ("Site") subject to the terms and conditions set forth in this document and any changes to this document made by VNUS from time to time (collectively, "Terms and Conditions"). By using the Service, you agree to these Terms and Conditions. If you do not agree to these Terms and Conditions, you may not use the Service.

Follow-Up by VNUS

You agree that VNUS can contact you following your use of the Service with surveys, questionnaires, and the like seeking information about your choice (if any) of treatment for venous disorders, your choice (if any) of medical provider, and related information. VNUS agrees that any such surveys, questionnaires, etc. will be reasonable in number and length.

Follow-Up by Physician

By checking the appropriate box(es) when using the Service, you agree to allow the corresponding physician(s) or medical practice group(s) to contact you regarding possible treatment for venous disorders.

Term; Termination

These Terms and Conditions are effective until terminated by VNUS. VNUS may terminate these Terms and Conditions with respect to you or any other third party without notice at any time for any reason. Upon termination all rights granted in these Terms and Conditions shall immediately cease to exist and you must promptly discontinue all use of the Service and destroy all copies of information obtained by you via the Service. In addition, in the event that VNUS determines, in its sole discretion, that you have breached any of the Terms and Conditions, VNUS reserves the right to terminate access to the Service and take any other action which VNUS, in its sole discretion, deems to be appropriate.
